NEBRASKA

Aging Services (HHS)

Area Agencies on Aging contacts and information.



Community and Rural Development Division (D.E.D.)

Recognizing the importance of community planning, the Department offers both technical and financial assistance to communities and counties. The Community Development Block Grant Program and the Nebraska Community Improvement Program both provide resources that seek to analyze community issues and through citizen participation develop plans that will assist in meeting community goals.



Credentialing Division (HHS)

This area licenses health related professions and occupations as well as health care facilities and services and child care programs.



Department of Education

The NDE is organized into teams that interact to operate the agency and carry out the duties assigned by state and federal statutes and the policy directions of the State Board of Education. The teams are organized around distinct functions and responsibilities that encompass leadership and support for Nebraska's system of early childhood, primary, secondary and postsecondary education; direct services to clients; and internal support to the agency. The department carries out its duties on behalf of Nebraska students in public, private, and nonpublic school systems.

Disability Programs (HHS)

The key components of the HHS System Developmental Disabilities System are: Service Coordination, Technical Assistance and Support, and the Beatrice State Developmental Center.



Medicaid and Managed Care (HHS)

The Nebraska Medical Assistance Program, also known as the Medicaid Program, is a program that is jointly funded by the State and the federal governments to provide medical services to those who cannot afford to pay for medically necessary services.



Mental Health Services (HHS)

The primary component of Nebraska's mental health services are the community-based providers. Many of these community-based providers are non-profit, private entities although there are private, for-profit entities as well. Increasingly there are private individuals providing services in communities. The other major component of the State's mental health system are the state run Regional Centers.



Nebraska Assistive Technology Partnership

The Assistive Technology Partnership links people with technology. The Partnership enables people with disabilities, their families, and professionals to learn about the latest innovations and identify funding for assistive devices and services.

NEW HAMPSHIRE



Bureau of Behavioral Health (DHHS)

The Division of Behavioral Health includes the Office of Community Mental Health Services, Mental Health Emergency and Crisis Services, the Office of Consumer Affairs, the Office of Homeless and Housing Services, New Hampshire Hospital and Glencliff Home for the Elderly. DBH manages contracts with agencies that provide services to persons with serious mental illness or serious emotional disturbance and to persons who are homeless or at risk of being homeless.



Bureau of Community Health Services (DHHS)

The Bureau of Community Health Services oversees grants to community-based agencies for medical and preventive health services, sets policy, provides technical assistance and education, and carries out quality assurance activities in its programmatic areas of expertise.



Center for Housing and New Community Economics

CHANCE's mission is to improve and increase access to integrated, affordable, and accessible housing coordinated with, but separate from, personal assistance and supportive services. CHANCE's purpose will be to offer alternatives to approaches that segregate, congregate, and control people with disabilities. The IOD will work in partnership with ADAPT in all aspects of the Center. ADAPT is a national organization that focuses on promoting services in the community for people with disabilities.



Department of Education (state)

The mission of the Department of Education is to provide educational leadership and services which promote equal educational opportunities and quality practices and programs than enable New Hampshire residents to become fully productive members of society.



Dept. of Health and Human Services (state)

Our Mission is to join communities and families in providing opportunities for citizens to achieve health and independence.



Department of Labor

The Department of Labor monitors Employers, Workers Compensation, and Insurance Carriers to insure that they are in compliance with NH Labor laws. These laws range from minimum wage, overtime, safety issues and workers compensation.



Division for Children, Youth & Families (DHHS)

DCYF staff provide a wide range of family-centered services with the goal of meeting a parent's and a child's needs and strengthening the family system. Services are designed to support families and children in their own homes and communities whenever possible, as long as safety is not an issue. DCYF is comprised of six major program areas and two key support service areas.



Division of Developmental Services (DHHS)

The NH developmental services system offers its consumers with developmental disabilities and acquired brain disorders a wide range of supports and services within their own communities. DDS is comprised of a main office in Concord and 12 designated non-profit and specialized service agencies that represent specific geographic regions of NH; the community agencies are commonly referred to as Area Agencies.



Division of Elderly & Adult Services (DHHS)

The Division of Elderly and Adult Services provides a variety of social and long-term supports to adults age 60 and older and to adults between the ages of 18 and 60 who have a chronic illness or disability. These services range from home care, meals on wheels, care management, transportation assistance and assisted living to nursing home care. Legal support services, advocacy for disabled adults, information and assistance regarding Medicare, and information about volunteer opportunities are also important support services provided in the community by DEAS.



New Hampshire Technology Partnership Project

The goal of the New Hampshire Assistive Technology Partnership Project is to increase access to assistive technology through the creation and support of consumer driven systems for the provision of state-of-the-art assistive technology products and services for citizens with disabilities in the state of New Hampshire.



Office of Community and Public Health (DHHS)

The Office of Community and Public Health is responsible for providing leadership and core public health capacity for NH citizens. The mission of OCPH is to assure the health and well being of communities and populations in NH by protecting and promoting the physical, mental and environmental health of its citizens, and by preventing disease, injury and disability.



Office of Health Planning & Medicaid (DHHS)

The Office of Health Planning and Medicaid is dedicated to the identification of NH's health care and social service needs through assessment of health care and social services delivery systems. OHPM has three organizational units with specific functional responsibilities: Health Planning and Research; the Community Grant Program; and the Medical Assistance (Medicaid) Program.

Statewide Independent Living Council (NH)

The Statewide Independent Living Council (SILC) identifies and sets priorities for independent living services and activities in New Hampshire, thus insuring participation by persons with disabilities in the planning process of the State Plan. Members of the SILC are appointed by the Governor and the majority are individuals with disabilities.

NEW JERSEY


Assistive Technology Advocacy Center (ATAC) of NJPandA

With a staff of over forty, it provides: information and referral technical assistance and training legal and non-legal advocacy, and outreach and education.



Department of Community Affairs (state)

Many programs are administered by the Department, which offer a wide array of financial and technical assistance to local governments, nonprofit agencies, and New Jersey residents.



Dept. of Education

The New Jersey State Department of Education will provide leadership for a superior education by utilizing multiple and diverse paths to success for all children in New Jersey.



Department of Health and Senior Services (state)

Services include: Family Health Services, Health Care Systems Analysis, Local Health, Long Term Care Systems, and Senior Affairs, to name a few.



Dept. of Human Services (state)

The New Jersey Department of Human Services (DHS) is the state's social services agency, serving more than one million of New Jersey's most vulnerable citizens or about one of every eight New Jersey residents. Among them are abused and neglected children; troubled youth and families; people with low income; and people who are mentally ill, developmentally disabled, blind, visually impaired, deaf and hard of hearing.



Developmental Disabilities Services (DHS)

The Division of Developmental Disabilities serves more than 30,000 people with developmental disabilities. These include disabilities such as mental retardation, cerebral palsy, autism, epilepsy, spina bifida, traumatic brain injuries and certain neurological impairments.



Disabled Services (DOL)(state)

Services include: Temporary Disability, Permanent Disability, Workers' Compensation, and Vocational Rehabilitation.

New Jersey Association of Mental Health Agencies

NJAMHA is the only trade association in New Jersey that represents the needs of nonprofit behavioral healthcare provider organizations. NJAMHA's membership consists of 125 community mental health agencies, which represent a total staff of nearly 20,000 behavioral healthcare professionals.



New Jersey Statewide Independent Living Council

NEW MEXICO



Agency on Aging (State)

This Agency is dedicated to promoting the independence and dignity of elders. As part of the Aging Network, our mission is to achieve the highest quality of life for older persons and their families by enhancing autonomy, health, economic well-being, community involvement and personal responsibility.



The Commission for Deaf and Hard of Hearing Persons

The Commission for Deaf and Hard of Hearing Persons was established by the New Mexico Legislature in 1991 (Laws 1991, Chapter 72) with seven members drawn from the community and agencies serving the community.It is the mission of the New Mexico Commission for Deaf and Hard of Hearing Persons to create awareness of, and to provide advocacy and ensure equal accessibility for deaf, deaf-blind and hard of hearing persons in the State of New Mexico.



Department of Education (state)

Department of Labor (state)

The New Mexico Department of Labor, a workforce development agency, is committed to satisfying the needs of our customers both job-seeker and employer.



The Governor's Committee on Concerns of the Handicapped

The GCCH is mandated by statue "to encourage and promote attention to the concerns of training and employment of handicapped persons in the state." In order to achieve this mandate, we must work in a variety of areas including employment, training, systemic advocacy, access (both physical and programmatical), housing, transportation, information and referral, etc. The GCCH is overseen by a committee of people with disablties who are appointed by the Governor.



Human Services Department (state)

HSD administers several large state and federally funded programs. Our largest programs are:Medicaid - (Health insurance for most low and middle income children, many elderly and disabled, and poor adults.), Temporary Assistance for Needy Families (TANF) - (Financial assistance and job training for poor families with children.),Food Stamps - (Food assistance for poor families and the elderly.), and Child Support Enforcement Services - (Assists custodial and non-custodial parents by ensuring that their children receive the financial support they deserve.)



Long Term Services Division (DOH)

The Long Term Services Division provides community based services to individuals living with a variety of disabilities. These individuals include those living with traumatic brain injuries, individuals with developmental disabilities, medically fragile children, the disabled and elderly population. In addition, the Long Term Services Division provides services to families of children with, or at risk for, developmental delays.



New Mexico Healthlink

New Mexico Technology Assistance Program

The New Mexico Technology Assistance Program (NMTAP) offers free services to New Mexicans with disabilities to help them get the assistive technology (AT)services they need.

Advocates for Children of New York, Inc.

AFC works in partnership with New York City's most impoverished and vulnerable families to secure quality and equal public education services. MS Upstate New York

The Upstate New York Chapter of the National Multiple Sclerosis Society serves more than 6,400 people with MS and their families in Upstate New York and supports national MS researchers working to find the cause, treatments, and cure for MS. Neighborhood Legal Services, Inc.

Neighborhood Legal Services, Inc. (NLS) is a not-for-profit agency located in Buffalo, New York. NLS provides free legal services to persons with low-income and persons with disabilities. It also provides a wide range of technical assistance and support services.



New York State Citizens' Guide

Welcome to the New York State Citizen Guide. This is where New Yorkers can find answers to State related questions, locate State services and access those services online.



New York State Independent Living Council

Promotes the independent living philosophy statewide, and provides support and technical assistance to the entire network of Independent Living Centers (ILC) in New York State which consists of 35 community-based, not-for-profit organizations run by and for people with disabilities.



New York State TRAID Project

Through a federal grant from the National Institute on Disability and Rehabilitation Research (NIDRR), Office of Advocate for Persons with Disabilities (OAPwD) provides statewide coordination of assistive technology activities for people with disabilities, fosters development of state and federal policies and programs to enhance availability of assistive technology, provides information, training, technical assistance and advocacy on how to obtain and use assistive technology services and devices.

Office of Mental Health (State)

Regional Center for Independent Living (RCIL)-NY

Our mission is to empower people with disabilities to self-advocate, to live independently and to enhance the quality of community life. Our services and programs are primarily of an advocacy nature (both individual and systems). We seek to overcome the barriers faced by people with disabilities who choose to live independently.

NORTH DAKOTA



Adults and Aging Services (DHS)

Provides programs and services to support older adults and persons with physical disabilities in their effort to live safely and productively in the least restrictive appropriate setting.



Children and Family Services (DHS)

The Division approves all applications for adoption in the state, contracts with three private child placing agencies for provision of adoption services for children with special needs, contracts with a private agency for the provision of services to refugees, administers the interstate compact on the placement of children, intensive in-home services, early childhood services, child protection services, Child Fatality Review Panel, family focused services, foster care services, services to pregnant teens, parent aide services, prime time child care services, wrap around services, the Children's Trust Fund and other children services. In addition, we license or regulate agencies or individuals providing care to the above populations to assure quality care and recipients' safety.

Hattie Larlham Foundation

A private, non-profit agency serving children and young adults with profound mental retardation and developmental disabilities. Established in 1961, the Foundation has expanded from its residential main campus in Mantua, Ohio to provide community care throughout the northeast corner of the state.



Ohio Statewide Independent Living Council

The Ohio Statewide Independent Living Council (SILC) is committed to promoting a philosophy of consumer control, peer support, self-help, self-determination, equal access, and individual and systems advocacy, in order to maximize leadership, empowerment, independence, productivity and to support full inclusion and integration of individuals with disabilities into the mainstream of American society.

Four million Americans are currently affected by Alzheimer's Disease. Aging Services Division (DHS)

Dept. of Commerce

Camp Attitude

We have acquired 34 acres of flat, riverfront land in Foster, Oregon. This wonderfully forested area will allow individuals, families, churches and youth groups the opportunity to enjoy outdoor living without the barriers found in many camps.

Abilities in Motion (CIL)

The mission of ABILITIES IN MOTION as a Center for Independent Living is to help people determine their own individual lifestyle, based on personal choice, from a wide spectrum of acceptable alternatives. We are located in Reading, PA.



Accessible Pennsylvania

You will find information on a variety of programs and services for Pennsylvanians with disabilities—such as programs for education, housing, transportation, vocational rehabilitation, employment, health care, home and community-based services, and child-specific services.



Carnagie Library For the Blind and Physically Handicapped

Internet accessible information on independent living and disability issues. This is a directory to resources in Pittsburg, Pennsylvania, and other international sources.



Community Resources for Independence, Inc. (Erie, PA)

CRI was developed by and for people with disabilities. Professionally trained staff, many of whom have disabilities themselves, provide peer support, services, and referral to existing community resources. Any person with a disability, their family, friends, agencies, or concerned others who would like assistance or information is encouraged to contact CRI to determine their eligibility. CRI provides most services free of charge to people of all ages. These services are provided in seven counties of northwestern Pennsylvania.



Dept. of Aging (state)

The department oversees many services and benefits to older Pennsylvanians - most provided through the 52 Area Agencies on Aging which were created in the federal Older Americans Act and Pennsylvania's Act 70. We are leaders of what is known as the state and local aging network, and partners in it.



Department of Education (State)

The mission of the Pennsylvania Department of Education is to assist the General Assembly, the Governor, the Secretary of Education and Pennsylvania educators in providing for the maintenance and support of a thorough and efficient system of education.



Department of Labor and Industry (State)

L and I administers benefits to unemployed individuals, workers' compensation to individuals with job related injuries, and provides vocational rehabilitation to individuals with disabilities. The Department prepares job seekers for the global workforce through employment and job training services for adult, youth, older workers, and dislocated workers. In addition, L and I enforces various laws and safety standards in the workplace and administers the Commonwealth's programs for community service by young Pennsylvanians.

The Departmentlicenses and regulates a variety of health facilities, such as hospitals, nursing homes, ambulatory surgical facilities and other in-patient and out-patient facilities. In addition, the Department supports outreach, education, prevention and treatment services across a variety of program areas. Office of Mental Retardation

The Office of Mental Retardation supports a comprehensive array of services and supports for people with mental retardation including: community residential and day support services which are either administered or operated by county MH/MR programs; contracted private residential intermediate care facilities for persons with mental retardation (ICF/MR) services or services in state centers. The mental retardation system is funded through federal, state and county funds.



Pennsylvania Council on Independent Living

The PCIL is a statewide, membership-based, nonprofit organization dedicated to the independence and empowerment of persons with disabilities. Founded in 1986, PCIL promotes the Independent Living Movement and voices the ideas of people with disabilities across the state.



Pennsylvania's Initiative on Assistive Technology (PIAT)

As specified in the "Tech Act", PIAT's priority activities include the development, implementation, and monitoring of laws, policies, practices, and organizational structures to improve access to assistive technology for all Pennsylvanians with disabilities and older Pennsylvanians.

Temple East Adult Day Health Center (Philadelphia)

We offer care and services in a medically supervised home-like setting for older adults. We have designed a comprehensive program of medical, social, personal, and related support services.
